Medicare Chronic Disease Management Plan (formerly Enhanced Primary Care)

If you meet the Medicare criteria for a Chronic Disease Management (CDM) Plan, formerly Enhanced Primary Care Plan (EPC), you may be eligible to have a portion or all of your Podiatry consultation paid for by Medicare. Whether a patient is eligible for CDM services will be assessed by the GP.

Patients who are eligible for Medicare rebates through the CDM are eligible for up to 5 Allied Health visits per calendar year.

Following an CDM consultation at East Gosford Podiatry, the patient will receive a rebate of $58.30per visit (rate as at 1 November 2023), which will go into their nominated bank account that is registered with Medicare.

Do I need to pay for treatment or the gap?

Yes, you will be required to pay the full consultation fee on completion of the appointment, after which the Medicare rebate can be processed on your behalf. Medicare usually rebates the funds within 24-48hrs.

What referral form do I need to see the Podiatrist through the Medicare Rebate Chronic Management System?

The ‘Referral Form for Individual Allied Health Services under Medicare for patients with a chronic medical condition and complex care needs’ is required to be completed by the referring GP and sent through to East Gosford Podiatry or brought in with the patient at their appointment.
